An old distress signal reaches the Enterprise. Following the source they reach an unnamed planet. Kirk, Spock, McCoy and Yeoman Rand beam down and find themselves in the hands of children, who call themselves "onlies". A virus has caused them to live long and age slowly. But when they reach puberty they die painfully and quickly. Being some hundred years old the children still behave childlike and even mistreat the landing party by throwing stones at Spock, kicking and beating Kirk and kidnapping Yeoman Rand. Spock becomes a carrier of the disease, while the others are immediately infected – all cannot go back to the ship. To add to the inconvenience the children steal all communicators. 

McCoy finds an old lab and research notes about an attempt to prolong youth and life gone wrong. He and Spock manage to work on an antidote which McCoy tries out on himself. The cure is found and the Federation will decide how to care for the children and how to use the planet.
