King Selim III of Qamadan is held prisoner by his younger brother, Prince Samadal. Together with Colonel Hatafis the prince maintains power over the country torturing their opponents. The IMF is to restore King Samadal to the throne. The king is officially on retreat when physician Phelps, actress Lisa and oil magnate Takis Demetrious (Paris) arrive to meet him. The prince is meeting them instead. While dining with Lisa he collapses (with a little help by Lisa and a bottle which self – injects the prepared ingredients into the whisky) and Jim diagnoses uremia. 

An immediate operation is necessary and the prince agrees because he is suffering under severe pain: A kidney transplant - from a donor who is closely related. King Selim is brought unconscious into the theater and Jim performs the "operation" under observation by Colonel Hatafis and his men. Barney and Willy operate from underneath the theater swapping the bodies and disguising each as the other. Jim is assisted by one of the local doctors, Dr. Labashi. The plan works because Paris takes his place. They brought enough red fluid along to dip the instruments in. Hatafis learns from Lisa that the prince will align with Demetrious and about their plan to get rid of Hatafis. Hatafis decides to assume power by killing King Selim – or rather the man he thinks is King Selim to later blame the prince for the deed. He succeeds in killing the man while Lisa helps Paris to change from Dr. Labashi to Paris. They leave the room of the real king Selim who is restored to the throne.
